Questions to ask before choosing a plan
- Doctors and hospitals: Are your preferred providers in the network? What changes if you receive care outside it?
- Costs: What are the premium, deductible and other out-of-pocket costs? Are there benefit limits?
- Prescriptions and services: How does the policy handle the care you expect to use?
- Eligibility: Is health underwriting required? Are there exclusions, waiting periods or restrictions involving pre-existing conditions?
- Timing: When can coverage begin, and what must happen before an application is approved?
Understand the type of coverage
A PPO describes a provider-network arrangement; it does not by itself tell you every benefit or limitation. Review the actual policy and benefit documents. Private coverage and supplemental benefits can differ from ACA Marketplace coverage. Ask which type you are considering and how its protections and limitations compare.
